Return of statement showing in respect of each Poor Law Union in England and Wales (1) the number of certificates of conscientious objection under Section 2 of The Vaccination Act, 1898, received by the vaccination officers, which were dated between the 1st day of January and the 31st day of March, the 1st day of April and the 30th day of June, the 1st day of July and the 30th day of September, and the 1st day of October and the 31st day of December respectively in the year 1899; and, (2) the number of certificates of successful primary vaccination received by the vaccination officers during the periods between the 1st day of January and the 30th day of June and the 1st day of July and the 31st day of December respectively in the years 1898 and 1899

II. Memorandum on the steps specially requisite to be taken in places where small-pox is prevalent

Return showing, as regards each port or other sanitary district in England and Wales (exclusive of any district wholly or partly in London), in respect of which, during the year ended the 25th day of March 1905, cases of small-pox have been reported to the Local Government Board, the number of such cases; and also, where the number reported has exceeded 100, the estimated amount of the expenditure incurred by the sanitary authority in consequence of the occurrence of such cases
